Under the general direction of the Lead Nurse, the Clinic Nurse RN with Geriatric Emphasis provides high‑quality, patient‑centered nursing care within an integrated ambulatory clinic. This role supports a geriatric‑focused patient population while functioning as a key member of an interdisciplinary team that includes medical providers, behavioral health, pharmacy, care management, and support staff.
The Clinic Nurse RN delivers skilled, comprehensive nursing care in alignment with Blacktail Health’s mission, vision, and values, with a particular emphasis on coordinating services for older adults across multiple care settings. This includes supporting transitions of care, communicating with Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), and other long‑term‑care environments, and ensuring accurate, timely follow‑up on provider orders, care plans, and medication changes.
The RN uses clinical judgment, critical thinking, and patient engagement skills to support chronic disease management, Medicare wellness workflows, preventive care, and patient self‑management goals. Responsibilities include performing nursing procedures, facilitating team‑based daily huddles, reviewing and reconciling medications, coordinating referrals, and maintaining accurate electronic health record (EHR) documentation. The incumbent also participates in geriatric care initiatives such as functional assessments, fall‑risk screening, cognitive screening, and advance care planning support.
This position requires strong communication and collaboration abilities, the capacity to work both independently and interdependently, and comfort navigating both in‑clinic responsibilities and off‑site coordination with long‑term‑care partners. This position ensures continuity of care for geriatric patients and supports an efficient, patient‑centered clinical flow consistent with Federally Qualified Health Center standards, regulatory requirements, and evidence‑based nursing practice.
